Rev.: Peter I standing right in Roman armor, cloak and laurel wreath, holding scepter in left hand and receiving a city crown with his right hand from the personification of the island of Ösel (Saaremaa), shown as a kneeling female figure in a shell crown, holding a scroll with the plan of the fortress.

This medal commemorates the capture of Arensburg (Kuressaare) on the island of Ösel (Saaremaa) by the Russian army on 15 September 1710 during the Great Northern War. The event marked the consolidation of Russian control over the Baltic territories, as the Swedish stronghold on the island surrendered to Peter I.
