Silver hallmarks on reverse: In the center: St. Petersburg district assay office mark (female head in kokoshnik, facing right, "84" zolotnik standard), and maker’s mark ВД and on the inner side of the clasp, hallmarks of the same assay office in a circular shield (female head in a kokoshnik, facing right, with a dot under the chin); on the ribbon beneath the Imperial crown - two Swedish import mark: three crowns in an oval and the letter "S" in a hexagon.

Obverse features the inscription in Church Slavonic:
ВОЗЛЮБИШИ БЛИЖНЯГО ТВОЕГО IАКѠ САМЪ СЕБЕ
(“Love thy neighbor as thyself”), a biblical quotation found in both the Old and New Testaments.
The badge was awarded to both male and female members of the Russian Red Cross Society for meritorious service during military operations and public disasters, or for long-term valuable contributions in peacetime, as well as for a one-time donation of no less than 5,000 rubles. It was worn on the left side of the chest.
